Thank you for choosing Malinski Dermatology. In order for us to provide the best care for you, it is very important that you bring the following items to your visit:
- Photo ID
- A list of your current medications
- Any and all lab results
- Insurance cards
- Medical records from prior visits that you think may be helpful
- New Patient Forms (please click the form below to download)
Dr. Malinski understands getting your insurance questions answered is important. Our office can share which insurance plans we work with, but since plan details vary and may change regularly, we are unable to provide what procedures are covered, co-pay information, or other details. Malinski Dermatology accepts most major insurances. Please contact your insurance company directly to confirm plan details and provider participation. We are currently still in the active credentialing process with several insurances meaning that new insurance plans are being added and accepted every week. Please bring your insurance card(s) and ID with you to each appointment. It is important to have your correct insurance information on file to help us correctly bill your insurance company.
